Rally should work with the 7 speed Campag Syncro index shifters, but not with the 8 speed shifters (either Syncro 2 downtube or Ergos). I've tried it; when 8 speed came in, the amount of cable being pulled by the levers was increased.I have one of the Campag MTB long cage mechs (Olimpus --- http://velobase.com/ViewComponent.aspx?ID=00C6A455-D259-483D-84D9-32B124C656BB&Enum=108&AbsPos=48) in perfect condition, but I know it doesn't work with 7 speed index and have been told that it doesn't work with 8 speed either. However, I could check the latter and let you know, since velobase claims it should.

Ok so Regina Americas are a no go for indexing.
I've used (and use) America 6 and 7 speed with Campag Syncro index levers, and have not had any problems. I use period Regina chains, including the hollow pin ones. There's a Regina Syncro 90, which I also use, but it doesn't work any better than the America for me.
I've not tried it with Shimano, but I don't see why it wouldn't work.

Campagnolo double chainsets up to the late 80s can fit a minimum inner ring of 42*, so you can't get a 39. From the late 80s on (Athena, Chorus, etc.), you can have a minimum 39.

Not white, I know, but they are 44. I take 10s, and these are too small for me, so you're probably right with Sidi 44.

^^^ For the record, NR, SR and N. Gran Sport rear mechs work just fine with 7 speed syncro levers. Been (and still am) using them in this way since early 90s (1975 and 1982 NR; 1977 SR mechs, Regina 7 sp. freewheels, various chains). There is no reason they shouldn't work with the original Campag 8 speed as well (5mm sprocket spacing), but I've never had the gear to try it out (7 speed still "modern" to me...)
