you take two photos, one of the bike in the shot (being held by hand) and another photo of just the background. You then overlay them in Ps and erase the arm and hand leaving the background photo to show through. tricky to explain :)
I wouldn't worry about shims tho, I've had a nitto one for ages and its completely fine. they may cost quite a lot considering how tiny they are, but saves you buying different bars or stem.
