Has anyone used these before?;
Shimano SH-MT22 Multi-use touring shoe
I've got a pair, mainly because there the best fit I could find, and they were dirt cheap on trade (Shimano mark-up ftw) Admittedly they do look silly, and feel a bit cheap, but they are comfy, easy to walk in and a good starting shoe for someone just getting into clipless - No point in splashing out on a pair of sidi's until you know what you really want from a shoe

Don't necessarily need an ITM one, just like the look of them. Why would it be so hard to find though? My bars are ITM and are 25.4, and I allready have two ITM quill stems with 25.4 Clamps, albeit shorter.
Those stems you link to are both nice, but way out of my price range. I think i've managed to find myself a nice NOS GB one anyway.
