- Sounds like the locknuts must be moving relative to the frame. This could ne caused by
a) the nuts not being done up tight enough - how long a lever are you using?
b) wear on the frame dropouts.
c) the locknuts on the axle usually have knurled bits to grip the frame but they can get smoothed off.
d) your transmission is totally worn out.
e)You are just hellishly powerful and should enter the world track champs in Poland in two weeks.

Ok. There is a hidden question in there. The fork, if it matches the frame, will be 27.0 mm and fork race will be 26.4mm and that's 0.6mm of a problem!
Pushing the headset in the frame might be tricky. If you buy an alu one you might distort or break it trying to get it in due to the high forces involved. A steel one might go in with lots of grease but you will flare out the headtube and I've seen them crack before...
The fork race is your main problem....
You could try buying a 30.0 /27.0 headset and then you have solved the problem without cutting tools.

This is what I would do:
By the chainset you like, fit a bottom bracket that you want (recommended for that chainset), and then space out the axle at the rear and redish the wheel to suit.
