Tried long and hard to find an answer to this on the internet but couldn't. I guess knee cartilage/meniscus injuries are fairly common among cyclists. I am about 75% sure I will have to have surgery (arthroscopy), or at least that surgery is the best option. My question is: what is the waiting time on the NHS to get knee arthroscopy surgery? I am basically out of action for all physical activities indefinitely, and it's killing me not knowing how long I'll have to wait. At the moment I'm going through the motions of physiotherapy. I know I don't need physio, my knee is/was fucking strong, I can feel the muscle strength and the ligaments are back to normal too; it's just fucked up inside. I guess they just ram this physio BS down your throat to try and minimise the amount of surgery they have to pay for.
Sorry to hear your experience. The NHS have an 18 week target for you to commence the treatment deemed neccessary for your condition. If you have seen a Consultant Orthopaedic Surgeon then you are almost there. You also have the right to choose the location for your surgery, along with any secondary care provision, so don't feel that you have to stay put if you are not happy.
As a Physio I'm sorry that you feel you are receiving BS treatment. A large number of people, elite athletes included, can often perform at the highest levels without surgery but if conservative treatment is not beneficial then I'd discuss this with your Physio.

If you are healed up with ongoing pain and Physio hasn't helped then a corticosteroid injection can work well to relieve symptoms, albeit sometimes temporarily. A surgeon I'm not but if you need some hands-on work, specific professional advice or would like to discuss and arrange injections please free to PM me.

The UK can be overly-conservative at times with management but many people, particularly those who are sedentary or not too fussed about full end range high-level function of the shoulder-girdle are happy to wait-and-see.
Metalwork provides immediate stability and a 'mesh' for bone growth. It doesn't mean that you're shoulder will be perfect afterwards. Most long-term issues will come from disruption of the acromio-clavicular joint surfaces with associated pain and crepitus with over head or twisting activity. Manual therapy can be useful for addressing these factors but cosmetic appearance is another matter with the typical 'step-deformity' a common issue.
Long-term, having metalwork in situ isn't ideal either. The collar-bone is designed to break on impact to protect other structures, bit like a car bonnet crumple zone, and if you were to have a future crash with metalwork in place the outcome could be more complicated.

Post-operative rehab for ACL reconstruction is fairly standardised and limited only by the graft compliance. You should be able to return to work well within a year. Normal guidance for athletes is running at around 3 months, return to sport at around 9 months (ranging from 6-12) but you would be physically able to work much earlier but perhaps not squatting / bending.
You will have to commit to your rehab if you want a successful outcome. I'll PM more specifics.
