Anyone read Zodiac by Neal Stephenson?
It's got a nicely worded description of the main characters cycling, essentially characterised by him saying that he rode as if wearing a luminous pink jumpsuit, with all the drivers in the city having been told that there was a $50,000 bonus for knocking him off.
i.e. his paranoia had progressed beyond "assume they have not seen you" to "assume they are trying to kill you".
The issue with cycle lanes and so on, as pointed out by Splat, is that they can put you in a section of road where other road users flat out do not expect you to be, so they don't look, with the results in the first post.
Personally I try to avoid them, as they do make drivers tune you out of their road view.

I cannot look at eBay at work, but if it is a blue and black 58-62cm Fort Aero Track from (I think) 2002, and being sold by a chap in Manchester, then yes.
