Pretty much exactly that - down and up the steepest parts only. It's the gradient that counts in this challenge, plus proximity of the next lump.
No idea what the record is, but I'd be looking for a few long steep climbs on the same hill, switching roads at the summit rather than the base, assuming they're closer together at the top. I've plotted a quick Leith Hill route that does that to an extent:

I reckon there's another couple of hundred feet to be had, but no more round these parts; only the one decent ridge between Kingston / Wimbledon. I'd be interested to see how much more can be achieved elsewhere.
