I wouldn't want to speculate on what happened, but there is an issue with people swinging across without looking properly from the other direction to get into the entrance, and pulling out into the bus lane without checking properly too, I go through Ladywell to avoid this section of road now.
Yes, let's avoid speculating, but I've certainly noticed the number of vehicular entrances there. As I don't ride there often, I've never seen a problematic situation, but it doesn't surprise me that it can occur.
