Surly Disc Trucker fork with a Avid BB7 caliper and a Soma Porteur rack. The rack attaches to forward facing eyelets so you don't have to mess around with axles supporting weight (and the resulting hassle each time you remove a wheel). Original brake pads still have a bit of life left (though I could never stop them squealing). Comes complete with rotor.
Steerer is 1 1/8", 20.5cm long. Brake is a a mountain bike BB7 (i.e not compatible with standard road levers). I've had to cut down the extensions for the rack to lengthen the struts to reach the eyelets.
