Italian lasts are notoriously narrow. I wear a 46 SIDI, but my actual shoe size is 44 (although my arch is actually a 46, but that's another story).

Also if you have a bike fit and they need to insert wedges... then a bit of extra space can help, but that of course is dependent on you needing such things.
My old sidis no longer fit as my feet have spread out from doing lots of yoga, so gone from 46 to 46.5

condor had some lady size sidis for 18-20 quid the other day when I went in.
They looked NOS from 5 or so years ago. Also had 10 inner tubes for £20 and bartape for £3.75.
Christmas bargains to be had.
and padded bike bags for £35.

I found that tip-toeing around in them in the bike shop is not a good indication of comfort on the bike. They really need to be tried on pedals, if they will allow you, so that you can work your foot through the full range of motion that it might experience on the bike. What I thought was comfy for walking about in turned out to be v. uncomfortable riding so I ended up selling mine and going half a size larger.
The insoles are a bit rubbish (thin) as well. Even if the shoe feels a bit loose on the foot a decent insole can make the difference.

Good point about the insoles. I don't really get it. £120 & you get some nasty piece of plastic that probably cost about 2p. I put a proper pair of insoles in my Sidi.

Definitely try them on first, I bought a pair of 41's off ebay about a month ago and they were proper tiny, in fact they probably would have been perfect for you, but I flogged them to Cam's girlfriend Hellen in the end

Cyclefits sidas footbeds are awesome. Even better than 531 which would be a shit footbed.

Whatever fits your feet is fine (see what I did there?). Ideally, a proper foot-fit, but not everyone is willing to pay for a professional to do that.
I use Think fussbetten, (sp?) FWIW.

Didn't. They aren't fitted, they are just the insoles from my favourite shoe company.
