I would have thought Murts that you measure from a line perpendicular to the stem centred on the intersection of the centre line of the quill and the centre line of the stem to the centre of the clamp...
Surely that would be complicated if quill stems don't have a standardised 'quill' measurement? Do they? Would it not be just as useful to measure the effective extension of the stem from the frame?

Then some different length stems would have the same extension dependent on their angle and would place your bars higher or lower for the same extension.

But isn't that why angle of rise/drop is usually given alongside stem length? But then I guess each time there was a different angle of rise the stem would be a different length. Argh. This is headfuck.

A note on sizing: Nitto’s method of measuring the length of the stem extension is a bit different than most. Compared to other brands’ sizing, their stems are about 0.7 cm longer than the listed size. A Nitto 8 cm stem will be approximately 8.7 cm long using the more conventional measurement.

Note: Because Nitto measures the extension length through the center of the extension rather than the top, their sizes wind up being a little smaller. So, a 8 cm Nitto stem will be the same length as an 8.7 cm stem measured as most other companies do. Generally, you can buy one size shorter than usual.

Damn, I've just measured a Cinnelli 1A, comes up about 5mm short on an 85, so I was measuring Nittos right everything else wrong.

In fact the Cinelli is from the centre of the quill bolt to the end of the clamp not the middle, head explodes.
I was a fool to believe in standardisation of any part of a bicycle. -
