The travel survey thing was an online fill in, you wouldn't have got a registration email if you didn't fill it in (or so it said on that email). The reg form has a thing on the back to declare how much you're giving to charidee, you have to tick a box on that and take that with photo id (not necessarily passport) to the Excel, I'm guessing your blue/blackness will be allocated there.
Anybody think the organisers are taking this way too seriously? It's only a bike ride for fuck's sake.
