forearm, elbow is a penalty.
shoulder, with intention to "attack" someone (like you see in icehockey a lot) is a penalty.
shoulder to shoulder in a duel is fine.
I played against a US guy in berlin last year on the far right field, guy played with alexis and kev. He constantly forearmed me and my teammates. When I had the ball next the concrete "boarding" of the court he elbowed me into that wall, this guy was really playing dirty, his teammates didn't.
