What you're looking for is a 135mm wide hub with freewheel thread on one side and disc brake on the other? It doesn't exist. The screw-on freewheel went redundant way before disc brakes became popular on bikes. You can either convert a standard 120mm fixed/free flipflop hub with a longer axle and space it out, or just take the easy route: a MTB hub with Velosolo cog bolted on instead of a brake disc and a sprocket and some spacers on the freehub body. Easy as pie, you just have to look at pictures to know how it's done.
