Is the frame Italian threaded?
If so BBs unwinding is not uncommon to say the least, its s stupid thread. Best to swap from anti-seize to locktight if its Italian.
Also, miche bottom brackets are rubbish, so it may be best to remove, bin and replace.
Oh, and your chainline should be perfect, as the miche BB is adjustable, so there is no excuse to have an off chainline. :-)
