    twice she failed to pay attention, one time almost caused an accident, the 2nd did cause an accident

    As a cyclist, you are the superior party--your vision is better and you're in a much better position to anticipate others' mistakes. As David said, I imagine that she was trying to get away from you. You really need to realise that a lot of people, men or women, are quite frightened in their cars. They're easily rattled.

    You obviously know that the correct course of action in a situation like this is not to proceed unless you can make eye contact with the driver. If you can't do that, you have to assume that you haven't been seen, and wait.

    You are of course most welcome to stand on ceremony regarding the division of responsibility, but it's only going to continue leading to situations like this. As I keep stressing: A lot of these people need help--they have often been led to believe that their car is the best mode for their journey when that isn't the case, and as a result they are under pressure from wasting time, effort, and money owing to calling in insufficient transportational utility. You can help them in many small ways--do it. Otherwise, they'll remain frozen in paranoid prejudices.
