i'm going to pretend that you don't know anything about bike, so don't patronized by what i say.
are you using two spanners ????? by tightening the the cone against the retaining bolt by turning both in opposite directions ? (on the same side of the axle)
The problem should never happen if you tighten the cones correctly like i said above.
