is the brake cable old??? is it damaged? ( it should be clearly visible), are all the nuts,bolts screws etc. fully tightened??
then check and make sure that the brake cable is not loose in the lever and fits in flush and tight.
My guess is that the cable was not properly tightened and thus it pinged off and when you redone it you didnt re tighten the cable.
