and if, like the one I hit, it is not at the side of the road, and is obscured by cars then take pics of that too. If the hole has suddenly been repaired since you hit it still take a photo. If it has a very sharp edge make sure that is clearly shown in the picture. Take several photos of your injuries too.
The rest, witnesses, bike shop estimates for damage and such, is much like any other accident. I had a lawyer friend draft all the letters for me, took about 6 months I think.
