I guess it varies between makers and manufacturers but from experience with both of Revelate's full framebags offerings: the mountain version gets quite a bit wider towards the front whereas their full road frame bag is uniform width. The road version has a horizontal velcro divider too, which helps (a little) with over stuffing.

I don't think so. I've got a mix of custom and off the shelf bags though and specified skinnier for my road bag.
The issue is more likely that MTBs have much more sloped top tubes, so your knees are nowhere near the bag when riding. Whereas road and gravel bike top tubes tend to be or approach horizontal and put the bag closer to your knees.

Really shit random question but are gravel bike frame bags typically thinner than MTB ones?
I am often catching my knees on my revelate one which originally fitted my Stanton Sherpa MTB
