Even better is people doing at the top/bottom of an escalator.

I saw a proper pile-up at the bottom of the escalator at Victoria once. Someone with a suitcase stepped off the escalator, took one step forwards and stopped. The person behind her had nowhere to go and went straight over her. Then the escalator just kept feeding people into the pile.
