The rubber should come out in my experience. There's a Cannondale spares website if you muck it up.
Strong magnetics really help as well.

Ha! The grommets do come out. Now I get it.
However to keep the cable tension I seem to need the whole length of cable from the lever to the caliper arm to be concealed.
I do not have enough spare cable now, having cut the bits to fit at either end.
