I strongly suspect the driver was on his phone (which is perhaps why the incident is still being investigated by the police). Driver just drifted straight into the cycle lane. Fortunately the whole thing happened at no more than 20mph.
@GoatandTricycle I've given more details about my positioning (directly behind the incident) in the rest of the report. Still suspect nothing will come of it.

Witnessed an incident between a car and a cyclist about ten days ago. Driver stopped; cyclist didn't seem too badly hurt. I stuck around until the police arrived, gave my name and address to the officers as a witness, then cycled home and thought no more about it.
Yesterday, a 12-page form arrived through the post, so that I could supply a more detailed witness statement. The last page of the form asks you to . . . draw your own collision! (Actually someone else's collision in this case. They even ask you to sign your artwork. I obliged. Obviously.
But I did refrain from adding turd. Sorry everyone.
