hey @tommmmmmm, my view is that the Rider Down thread is the most appropriate place to establish exactly what happened in each and every case (the facts), so that we're clear on which scenarios involve fatalities, and how these can be avoided;
appreciate your empathy for the other rider, but still would like to know more about how such a collision happened,
{only with memory of one previous incident whereby a person on a bicycle collided with somebody such that it led to death, recall that person was riding in such a way to endanger the pedestrian public}
still would like to know exactly what happened in this case, was it just one of those things?

This is hardly the place for speculation and suggestions, there is a protocol in these threads for a reason.
RIP rider, thoughts with his family.
The other rider, like most involved in road traffic accidents, presumably also did not set out to cause someone's death, and presumably also isn't in a great place at the moment.
