Shitty square tapper bb, no spacers, so guess that's out.
Thanks for the alternate options @miro_o the cause of the problem is running the kiddy cranks on the drive side to the front cranks, if I put a double at the rear offside and have the kiddy cranks just going straight down it should be fine... Should be...

BB ones attach behind the driveside BB cup but you'll need an e-type BB with with room there to put things.
